Community Health Improvement Week: HIDI Provides Tools To Assist Hospitals With CHNAs

This week marks Community Health Improvement Week. HIDI helps hospitals improve the health and well-being of individuals and communities, and advance health equity through access to data. Providing specific information related to health factors and health outcomes in the places where patients live equips communities and organizations with the information necessary to act and create a healthier state. HIDI offers a wide range of applied analytics and custom study applications leveraging data, tools and curated content available in our secure cloud-based HIDI Advantage Analytics Workbench. We partner with select industry partners with distinctive capabilities to complement HIDI services and expertise. 

exploreMOhealth is a community health needs assessment platform created to provide ZIP code level rankings across major health behaviors and health outcomes to illuminate high-risk communities at the subcounty level. Developed by HIDI researchers and Washington University School of Medicine, Missouri ZIP Health Rankings combine with County Health Rankings to create one of the most unique health-related datasets in the country. The site was launched in 2018 with support from the Missouri Hospital Association to help hospitals and community health stakeholders optimize the effectiveness of community health improvement interventions.  

HIDI also partners with Tennessee Hospital Association to develop their state-specific resource ― exploreTNhealth. Many THA member hospital facilities use the data to update their CHNAs and support collaborations with public health partners in the communities they serve. Like the original Missouri model, exploreTNhealth is based on the population health model used by County Health Rankings & Roadmaps, developed by the University of Wisconsin Institute for Public Health. 

These free, publicly available resources are intended to help communities, hospitals, public health officials and other stakeholders identify local health needs by ZIP code or county, and to develop health needs assessments and actionable plans to improve public health.
